4.2 head gaskets

Hello,im in the process of changing head gaskets on a 97E-250 4.2. This is the second time this year it is unreal.The first time it was done by a mechanic.Does anyone have any ideas of what might have went wrong,before i make the same mistake.This van only has 58000 on it.It was leaking coolant around both cylinder heads and lower intake.

Make sure You check the heads for warpage!!!!
I have an upper gasket set if you are in the market, I paid $191.00 for a felpro set right before My engine blew up.
