THe form I used was for FLA and it is located on the Attorney General's webste for the state of FLa. They have a very good section discussing the Fla lemon law.
do some research, lemon law in my opinion is a hassle. you have to give them 3 attempts to fix it, then a letter stating it is the last time, then you go to an arbitrator, then if they cant solve it then you go to court, and the problem has to still be a problem during the time of court blah blah blah.
I used magnason moss, its a federal law which applies to any product over $25.00 that has a warranty. your claim is that after reasonable attempts they could not correct the problem and have breached their warranty.
I also live in florida, and as we speak I have just finished a case, my wife had a hyundai tiburon and it was in the shop over 15 times in the first year. the sunroof motor was replaced 13 times and the window motors 3 or 4.
we settled out of court with hyundai and I traded that vehicle in with my old truck for my new f150. hyundai paid all lawyer fees and paid us as well. I would suggest you do your own research and see which way is best for you to persue. I live in florida and used Khron & Moss lawfirm, they are free of charge and you can find their webpage, there based out of chicago.
hope that helps, good luck.
