Clutch Problems?!

I own a 95 F150 with a 4.9 and 5 speed. Every time I go to let out on the clutch to change gears or take off the truck lerches noticeably it kinda does a herket jerkey forward backward motion the only way to stop it is to rev a little higher and ease the clutch out slow letting it sli quite a bit before grabbing. I'm relatively inexperienced with transmissions so any help is greatly appreciated.

You might want to search and/or post in the Clutch, Transmission, Differential forum and see if you find anything on this and maybe get more response??
