I was thinking of buying one of the dually conversion kits. JC Whitney sales one, It is just a hub spacer to bolt another wheel on the outside of the existing one.
thats a new one for me, ive seen a dually suburban before, and on a small dealer lot in marion here i saw last week a 82-88 series ranger with a 302 and dual rear wheels, but i had never seen a van done that way before.
i had one we built to haul the 4x4 to camp and it was cool turned alot of heads think its still runnin around wi. started out with a 76 1/2ton shorty 300/3spod swap for 390 c6 used the j.c kit simple and for the wheel wells they came of a 88 chev 1ton we had to weld one of the side doors shut cuzz of the wheel well bein to long but still had full use of the other doors(no slider swingin sides) painted it maroon w/yellow bumprs had lund visor and front spoiler and all the trimmins oh ya we had to put in a set of 2500lb overloads to give ya an idea what it would haul we cleaned it ot and had 8 engines inside all tore down with a car trailer behind with a 8ft bed standin infront 29 9"rears 4 c6's and more tires and wheels than i remember 37 mile to a swapmeat the only thing it didnt do very good was stop,the wife hated it lol

The roll over factor is greatly reduced, and as you may know, when one of these babies is upside down with the contents spread out over 3 acres, it makes the news.....
So yea, figure out a way to make it look right and you could be in the retro fit biz in no time.....
I've only seen an experimental for the dually on the E350. Looked really nice and the fenders only extended slightly (6") beyond the current body. You can buy the adapter kits and they ARE worth the money if you are trying to prevent rollover. You can get the rear wheels only but you will need to get 4 new rear tires (slightly narrower) to prevent rubbing on the inside. I called Quigley and they sent me to a place in CA. Forget the name, the down side was that I would need fenders and I just didn't like the Quigley fenders. I couldn't find any other source for the fenders. If you know of a fender source, post it. Most places provide pickup fenders, but not vans.
