Tranny "fluid" should be changed every 25-30k, being yours has so many miles and unknown if it had ever been changed, take it to a tranny spec. you trust and ask him what he says. Personally I would do it, torque converter and all. I recently just had mine changed and we found some metal on the mag pickup, probably a planetary gear going south, no problems yet but indicators. I have 340k on mine and have changed it regularly. Preventive maintenance is cheaper than repairs.
Ford dealer would be good. Trustworthy trans. shop (not AAMCO!!!) would be good. Jiffy-Lube would be B-A-D!!! I wouldn't even trust those knuckleheads to put air in my tires!!!
