Put a generous amount of silicone in the ball opening and stick it back in the shaft. Should hold it for a good while but still allow it to be removed for transmission service later. Worked on a Ranger pickup for me.

I used a glue called "Welder" that I bought at WalMart. It worked great on the shifter **** and also repaired my inside door handle/arm rest.
You can buy a leather SHO **** from Texas Mustang Parts or several of the others. It is $30.00. But it has threads, so you would definitely have to find a good glue to hold it on.

I have a 96 5-spd Explorer and the same thing happened. I just used locktite superglue and it's been held on there for the last 4 years without a hitch.
