1991 F250 5 speed 75K (bought at 44K). Was loosing coolant, reworked headgasket, that fixed that problem. However, I suspect that it may have been a historical problem.
Oil pressure, after reaching operating temp and traveling for several miles, will drop into the toilet at idle (mostly sometimes). The mileage seems to make a big differnece. In the 30K that I have driven, I have only added about 4 Qts of oil total, it doesn't use much oil. I have managed to improve the oil pressure problem by including about a quart of STP type addative at each oil change, but the cure is inconsistent.
The engine does seem to rattle some on intital startup, but quiets down when the oil pressure comes up.
I would screw in a positve reading gage and check your pressures to eliminate a faulty oil pressure gage in the dash or a defective sending unit. On my 88 f 150 I have to tap on the temp gage to get it to read right, it sticks at the bottom,I really should replace it.
Although that noise on intial start up makes it sound like the bearings in the bottom end.

What grade oil you running? Try 15-40 in Summer and see if that helps. Agree with above as well -- with pressure guage that reads only H and L, you don't really know what is going on. General rule of thumb -- 10 psi per 1000 rpm is sufficient. You may have that much and no know it.
