Anyone played w/ 2 speed rear ends??

I have a 1994 f-350 2WD 7.3TD non Pwr stroke w ZF 5spd man trans crewy Dually. The truck is primarly used for towing 14.7K of goose trailer and boat. I have two problems. 1)1st or (low) isnt granny enough to pull the boat out of more inclined boat lanches...I have to slip the clutch in these instances and its taking its toll.
2)5th gear or (overdrive)is to tall(rpm to low feels like your lugging it about 2000-2200 rpm) and 4th is to low (pulls better than 5th but seem like your racing the engine rpms are about at 2900-3000)in trying to maintain 60-65 mph.
My questions are, I believe a spliter (2speed) type rear end would solve this problem giving a 1/2 gear (or lwr 1st) and 4 1/2 gear which i think would be great...Has anyone out there done this? if so who's or what rear end did you use??
And does any one know the rear axle ratio of my truck?(it says on the D>S> door tag "rear axle "45").
