It does sound like the rotors are getting weak, and warping under heavy use.
What year is your van? The models before 1992 came with straight vent blades in the rotors, which do not work as well as the 1992 and later models with the curved vent blades. However, the later rotors will fit onto the earlier vans. When you order new rotors, you should ask for replacements for 1992 or later models. They can be distinguished from the earlier models by being specific to the left and right sides. (The earlier straight blade rotors had just one model that would fit on either side.)
When you install them, make sure the correct rotor goes on each side, or you will overheat them even sooner. If you can see the vent blades as curved fingers, they should be pointing backwards at the top rotor.
I got my directional rotors with the curved vent blades from a company called Autospecialties. I forgot exactly where I ordered them from. Note that they also make cross-drilled and slotted rotors, but I would avoid those for a street car. They will not last as long.
