I have a 1997 Ford E350 Van with the 7.3 PSD and E4OD transmission. It also has the quigley 4x4 conversion.
All the shifts are hard, most noticed from 2 to 3.
It also been stalling alot from idle mostly in reverse. sometimes in 1st as well. I can't remember if this started after I changed the tranny filter, but I don't remember any feet on the filter.
Before I started doing mods to my truck, the tranny would shift as you described, especially 2nd to 3rd. The thing that helped the most was adding a chip, it made the tranny shift firmer and more precise. I am not a big fan of the oem tranny myself, I wish Ford had 5 speed allison. If you spend some money you can improve the shifts, check here www,bankspower.com- they offer a system that is called transcommand that improves the performance of your tranny, unsure of price. As far as your stalling imop I dont think its related to the tranny.

Other than the trans ,what do think of the 4x4 vans? Before I ordered my current rig, I was seriously looking at those. I was torn between the crew cab and the van, I think I should have gotten the van, but do love the truck I have now. How does it handle on the highway and snow if you see any?
I had a similar problem with my AT on my F350. During the shift from 2nd to 3rd it would only stay in 3rd gear for about 1 second and then shift to 4th gear. I noticed this would NOT happen during cold weather. I asked a oil change outfit about the fluid and they said that it takes a special AT fluid - Mercon or Merco. I had the fluid flush out and it improved some what.
