I have a 98 Expy and I'm getting ready to repaint the hood for an overspray problem. I noticed that I dont have a hood liner. Should I consider getting one? Do they really protect the paint on the hood?( from engine heat) or is it just for noise?

While I'm certain that they offer some protection to the hood paint from engine heat, that is not their primary purpose...when you're driving down the road, the airflow over the hood is probably sufficient to prevent the paint from being too baked by engine heat...and when stopped in hot weather, I don't imagine the liner would do too much.
The purpose of the liner, as I've read, anyhow, is twofold: insulate engine NOISE and help with engine fires. The little plastic grommet things that hold the liner on melt very easily...and if the engine catches fire, they melt and the liner falls on the engine, hopefully helping to smother the fire.
I'd say you probably don't need one for paint protection duty.
You can also make your own using heat resistance fabric from JC Whitney. The "plugs" that hold it in are available at most auto part stores. Well...not the big ones but more the small shops that specialize in auto body.
I did it to an old truck of mine for about 40 bucks.
