Sounds like possibly a stuck open injector, or maybe a vacuum leak. Exporers and rangers like to have bad O2 sensors as well, which could be your problem, but I'd check other things first. I doubt a plugged cat. Run some fuel injector cleaner, that usually cleans up hesitation. Since the injector pulse width is higher than expected, it is putting more fuel in than it thinks it needs, possibly due to bad O2 sensors, or a vacuum leak. If it's a mass air motor, I would clean the heating element in the mass air meter, which usually makes the vehicle run lean as well.
