What year truck? I think for 01 and 02 it was just some skid plates, and for 03 it's the paltes and some rancho shocks.

The Off Road Package as described by Ford: 
FX4 Off-Road Package $225.00
- Transfer Case and Fuel Tank Skid Plates
- Branded Rancho Front and Rear Shocks
- Steering Damper
- "FX4" Decal
I just went with the "skid Plate" option only for my 03 on order, The steering Damper comes with the Snow Plow Package, and the shocks I can do myself. Seems to me all thats left is the decal....expensive decal

I just received my truck and ordered this option. Also, limited slip axle of your choice and "off road" type 265 tires are part of it which add a few more bucks than the $225. The skid plates can be ordered without getting the whole "package" as mentioned. I have the steering damper also with the snow plow package. Man, does the truck sit up high in front. Now it's level. I recommend the plow package if you want it to sit up in front.

Mid to late 90's if I remember correctly....Transfer case skid plate, a skid plate for each fuel tank and the 4x4 off road decal.
(FWIW,I deleted the decal back when you could(96).It was a break even $$$ proposition on the decal delete option.)
