V10 idle

I’ve been working on my 2001 F250 with a 2V V10 for some time now trying to diagnose its abnormal idle. It will idle smooth for a bit then “cough” or “putt”. You can hear it in the exhaust and feel it in the truck when it does this. The truck has 237k on it. I’ve owned it for a little over a year now. Due to the higher miles, I started with replacing the following parts (all Ford or Motorcraft) after some diagnostics.

coils and plugs
fuel pump, fuel filter, fuel pressure regulator
O2 sensors per TSB 3-9-11
repaired exhaust manifold studs and gaskets
cleaned throttle body and MAF
PCV hose

I’ve smoke tested the engine for vacuum leaks and found nothing.

The only abnormal readings I’m finding on my scan tool is the spark advance and fuel pulse width.

The spark advance at hot idle should be between 17-23 degrees. My truck at hot idle is between 11-14 degrees.

The fuel pulse width for both banks should be between 3.8-4.6 mS. My truck is between 3.0-3.4 mS.

I’m thinking it might be time for a timing set due to the spark advance being out of spec. What are your thoughts? I really appreciate any suggestions / help. Thank you!
