I have a very very random issue where I won’t shift out at of 1st gear. 99% of the time the transmission shifts perfect with no slipping or other bad behavior.
did some reading at this seems like symptom of a bad lead frame.

There is a recall on this generation truck that covers reprogramming the control computer that sometimes fixes issues like this however, it is highly possible the leadframe needs replacement both of which requires a dealer visit for reprogramming so you could install the part if it needs it, but you'll never get it to work until it's been programmed to the system.
Call or go see your local dealer service dept and ask them to run an OASIS on your truck vin# with your symptoms and they'll let you know how to proceed. If you decide to scan the truck for codes related to this, DO NOT CLEAR THEM!!!!. leave the codes in memory for the dealer tech, he'll need to verify certain parameters to determine warranty eligibility, as in yes you might get it fixed for free.

Slightly connected: 2014 F150/4wd/5.0/245k Just had the Recall #24S37 recall done 7/17/24. under recall notice since I purchased new they had my address/contact. Couldn't find info on what it does other than "reprogram PCM to fix" issues. Afterwards had idling issues all the way home then took 6 restarts after dying to go 30ft. Limped it back to shop,(they kept overnight) verified symptoms and found DTC P115E- they cleaned TB and runs fine now (advised if symptoms return, replace TB). just an fyi.
Question for the more knowledgeable. If the reprogram reset the PCM, would the TB also have to "relearn"?

The pcm is a learning computer, it reads data from inputs to determine outputs, the TB is an input and an output in this case (fly by wire). It tells the pcm where it is, the pcm decides where it needs to be via other sensor inputs and sends that command back then watches what it does and adjusts on the fly so to speak. When ever a system reset, reprogram, etc is preformed, it's expected (they should have mentioned it) to perform differently for up to 500 miles, according to Ford that is. I've personally felt them straighten out and fly right within 10 miles of repairs on my own vehicles.
Cleaning the TB was likely also needed and not surprising why if you ever looked at the back side of the butterfly within.
