This truck that I bought has the fifth wheel prep package. I have done a lot of towing in my life but I have never towed gooseneck. When I look down into these holes three out of the four holes have shapes down inside and I can visualize how the hitch would drop down in there and then turn and lock into place. However one of the holes appears to be completely wide open. Is this normal?

Both of the holes in those pics look identical to me, sort of a double "D" shape.
That oblong hole below the puck hole in that first pick has nothing to do with the mounting puck hole, or anything to do with the hitch mounting or chain anchor mounting.

These are anchor points for a 5th wheel, right? Not a gooseneck. The gooseneck only sits on the center point, the kingpin, the larger circle mount in the center of the bed. I have a B&W turnover hitch in my truck and I do not have the D shaped anchor points for a 5th wheel hitch.
^^^ That’s right? Although you can get an offset gooseneck ball that uses two of the puck mounts to secure the gooseneck ball from rotating. Also when using the gooseneck ball a couple of the puck mounts are used for safety chain anchors.

THIS is probably the answer. When I look down into 3 of the holes I see "slits" or shapes down in there (two holes have ovals, one has an oblong shape). But the 1 hole (passenger front) is wide open and I can easily see the shock down under the truck when I look through it. I suppose I assumed that these slits or shapes are what the 5th wheel hitch would fall into and lock into place with. So you're saying those sub-layers of steel mean nothing, it's actually the top part of the hole where the hitch locks in. OK, in that case all of mine are the same then.

So, it sounds like you don't have the 5th wheel hitch that fits the puck system yet.
Once you do, all your questions will be answered, will be readily apparent when you see it.
While we are talking about prep package placement, is anyone elses prep package not centered? Mine is 1/4in off to one side and its correctly on the locating pins when i pulled the bed to look.
At least 2 threads in the towing section somewhere identical to this one. People thought something was wrong, people chimed in that something was wrong but every factory 5th wheel prep is identical to yours.
