I have had 2 truck now with the 4.6 2v, 4R75w, 4.10 axle combo and that seems to be the sweet spot for this motor, it has no problem maintaining hwy speed into a headwind and will even climb some grades without downshifting out of OD, but it's turning 2200-2300rpm at 70mph and I don't think it's ever done better that 15-16mpg.
All that said, the manual trans doesn't have as deep an OD ratio as the automatics so this truck shouldn't be that sluggish, I'd suggest you test fuel pressure because a partially clogged filter or weak pump are common with vehicles that have sat unused for a year or more.
Ok, I ran across this very issue a few years ago where the owner replaced the air filter the wrong way and that bullet nose plugged the air intake on acceleration, It'll fit both ways. Flipped it around and cured the concern.
I'll check that in the morning. I'm pretty sure I installed it right. I've installed hundreds of them used to work at a valvoline instant oil change for 12 years
