1982 E350 (assumed) cab and chassis now camper. Low power, no smoke. So many things on the fix it list, just wondering what to check first on the engine part of it.
If you're seeking answers to specific questions by all means post them here--IF you believe anyone has time or interest watching a lengthy video looking to deduce a question and provide an answer you're somewhat mistaken, obviously.

You know how EGR works and how to check the rate of ignition timing advance?
You can check an ignition advance amd have a vacuum gauge / pump?
Do you have a timing light? If you can say yes to a few of those questions you have a good start
What to check first? IMO the ignition timing. 2nd , have a smog test done and get a printout of the CO and HC numbers
That old van engine is most likely tired and is not going to run like a creampuff. Do the best you can with what you have

Not sure what you mean spews exhaust out the cracks, but that is bad
The exhaust manifolds are cracked? Gaskets leaking?
Try to get that fixed too
If the O2 sensor sees too much oxygen it has the processor send more fuel to compensate
Sounds as if your exhaust system has gone bad. You probably need to upgrade it and should probably start with headers and free flow mufflers (I use Gibson they are free flow and not loud). The engine should not be pollution issue plagued, as in 81-82 should only have had an egr valve, smog pump system. Think about getting rid of the carb and install an throttle body injection system (on youtube one mechanic installed one and was happy (It was his own vehicle, it was a holley unit). This is if you really want to fix issues but will incur some $$ being spent but if this is your desire it is still less than upgrade to new/newer unit. its you call
Check the muffins and O2 sensors or throw a $3,000 EFI system at it. Got it. I'll be starting with the muffins....
If anyone is curious, I am filming what I do to it, my 94 F350, a TJ, and a 1927 Ford on youtube. It'll be awhile before I cut the exhaust off and take a look but active on other things here through the winter. Here's a video I shot of the engine, someone asked what we were talking about and I couldnt remember if it was a 351W or 400m - first time I ever removed that doghouse!
Looking up mufflers on my zero dollar budget here. UGH. I'm not sure what to put on my xmas list. I definitely want free-flowing. I definitely do not want highway drone or raspy revs. Any suggestions? 2.5" 3"? It's already dual and no cats as you saw in the engine video.
Oh and - is there an audience for videos like these? I'm "this close" to meeting the minimums for youtube to start paying me. I would of course re-invest that money into projects like this - an EFI "Carb" conversion is on the wish list... Just need a big enough following to generate the view$.

If the stickers are still under the hood, it should tell you it's either a 5.8L 351 or a 6.6L 400. So you either have a 351C a 351W a 351M or a 400 For did not make a 400M
As for your changing to FI, Just my opinion, but leave it carbed, you'll be happier.
Your other statement, you want free flowing mufflers but you want them quiet. Well good luck with that.
I myself have never seen a free flowing quiet muffler, but if you want less restrictive but still have some rumble, Thrush or Walker are good alternatives and if you rework the exhaust and move the mufflers further back, you'll get less noise inside your cab area.
Got a set on my 350 chevy, sounds perfect, just don't know what brand they are! No drone, all baritone growl. If you watch the videos I made you'll see what engine it is.
