I have about an inch of slack in my accelerator Cable and am looking for aftermarket replacements. I have an 88 f250 with a 7.5L. I note there are two options for lengths 42.5" and 43.25". First of all why are there two options and second does anyone know which one is the correct length?

I would bet they each have a different "travel" designed into them. My guess is a throttle body or carburetor(if applicable) difference in the closed-to-open on the plates https://shop.broncograveyard.com/Thr...ctinfo/30796A/

where is slack being measured from? If you hold your throttle to open under hood, and have someone take a picture at the pedal, is that the inch clearance? Such that the pedal could be all the way down and your throttle not all the way open?

Yes, the pedal does not pull the cable enough to open the throttle all the way.
Look closely at the shape of the case end that attaches to the engine. There are two different mounting styles, the "square-ish" one with 4 mounting tabs is 43 point some inches, and the one with one oval tab is 42 point some inches. The length probably doesn't matter a whole lot, but the mounting tabs do.
