Heres a pic of my ongoing project truck:
I had a knock in my old engine while warming up also. It ended up being a bad rod bearing. I ran the engine for years like that. The oil pressure was a bit low, but it ran allright.
Hopefully I will be able to get a lot more miles out of it before a major rebuild since it does not leak and uses very little oil.

1. fuel pump arm on eccentric on the cam getting worn and making noise
2. timing like you said, but that shouldn't change that much, and at idle it'd take a LOT of timing to make it knock.
3. Rod side clearance. This is common and mine does it. The fix requires a rebuild and new rods or a new crank; dont worry about it if you find out this is what it is. A dropped pan and a feller gauge will determine this.
4. Hydraulic lifter going out; the solution is a new lifter and running in the new lifter on the cam. Ask if you have questions on this.
5. Wrist pin; this would require a rebuild, dont worry about it if it doesn't do it warm.
6. Piston slap; Worn pistons will rock in the bore and knock when they're cold. This is another strong possibility of what it is, to check if it's this, back off the timing (retard it) at idle. Set it about at TDC, disconnect the vacuum advance. If the noise goes away, you've found your problem. It'll run like crap while you're checking it, though. If this is what it is, dont worry about it, it requires a rebuild and new pistons. If it gets to the point where it does it warm, start thinking rebuild.
Edit: Dont forget to plug up the vacuum advance hose when checking #6! A golf tee with some grease on it works good.
