The lower column bearing is likely shot and causing the excessive gap between the steering wheel and shift collar. Very common on these trucks for the bearing to fail and cause the issues you have. A new rag joint may help pull things back into correct position temporarily but if bearing is bad will only be a band aid. It's not the job of the rag joint to keep column in correct position. Might as well do it now since column will need to be removed and you're likely halfway there now.

OK. Can you link me instructions on how to replace the bearing? I can get the column out but I need to know how to do the bearing. Do I need special tools? What parts should I buy, just the bearing?
I worked on my horn on and off for years until I found all the problems.
Some new brushes do not conduct electricity but can be fixed. I went around in circles until I figured this one out.
The brush contact that looks like bronze but failed the ohm meter test and when I tried to polish it, I found it soft and powdery.
I appears to be make from a sintered bronze power and a binding agent smashed together like a pill.
I clipped onto the wire just behind the big contact to hold the spring back and heated the contact with a propane torch to burn off the binding agent and kept heating it until red hot and smoking stopped.
Then I cleaned and shined it up. Now it works fine.
Missed production step - Hmm?
New rag joint on. New steering column lower bearing installed. There was nothing left of the old one except two races. There was no felt piece like read in some other posts. There is no gap at steering wheel now. Now I need to install the new horn assembly that I received from Dennis carpenter. The bracket is different than my original horn so I need to relocate it. Any suggestions?
I know this is an old thread, but let me chime in with a simpler fix.
The conductive metal ring pulls away from the copper brush due to the plastic wheel shrinking. Remove the three screws and shim the underside with metal #8 washers. The metal ring should now touch the copper brush and your horn will work again.
