I have a 97 f-250 extended cab. The bed is severely mis aligned and I’m wondering what could cause this. Truck is pretty rust and I noticed the bed is sitting on the frame on one side but cab seems to be in line with the axles.
Which side is the bed sitting on the frame? Driver side?
I would guess you either need new cab mount bushings or new rails under the bed. Possibly both need replacing. I had a bed alignment issue and new cab mount bushings fixed the problem but it wasn't nearly as bad as your Pic shows. Can you post a pic from the rear of the truck? Try to show bed to bumper and bed to cab alignment.

from the video I received from the owner (looking to buy the truck it’s a great deal but wondering if this could be why) it appears that bed is sitting on frame in the side that it is higher on. Makes me wonder if it’s bad suspension on driver side. One of the cab mounts does look horrendous. I will add some picture of bed to cab, cab mount, and bed mount Cab mount 🤮 Bed mount looks pretty bad too
