My truck has an issue where under any sort of acceleration it surges horribly, sometime to the point where my head is bouncing off of the headrest. Occasionally I will get bank 1 and 2 fuel ratio imbalance codes that randomly go off. Here is a list of everything that has been replaced so far:
MAF (2 times)
Upstream and Downstream sensors
Air filter
Fuel pump
Throttle body
Intake
Engine (2 years ago)
I think that's all

Is the surging coming from the engine or the transmission?
If you've changed all the above, it sounds like a classic faulty TPS (throttle position sensor) or could be a fuel delivery issue, most likely a clogged filter if you've already done the pump. First step would be to get an OBDII scanner to read codes pull some live data, especially when the vehicle starts surging.
Google ForScan and get a cheap bluetooth OBDII adapter for it.
