P209F Nightmare

I need my truck to pass smog, it did have a DTC P209F I looked into it and the def heater assembly failed, open circuit. I replaced it with a ford assembly verified that the circuit is now good. reset the codes in the computer drove the crap out of the truck and verified that all emission monitors have been completed using my Snap on Verus pro. I arrived at the smog station and get a fail for P209F. The check engine light is off, no pending codes, no freeze frame data saved. I cannot for the life of me find a DTC in the computer using multiple different scan tools any idea what's going on? Has anyone else ran into this problem?
So, I replaced a def heater assy in a '14 a couple of weeks ago. I cleared the codes, gave it back to owner and he ran it for about 3 days and got the code and reduced power issue again. It came back and I dug deeper into the wiring and checked the heater assembly again. It tested good. I put the heater assembly back into the truck and cleared the code again. Now the only difference is I used my Snap On Verus Pro when I cleared the code. The second time I used Forscan. It has been fine for the last 10 days or so. His check engine light did come back on and it was the P04DB code for the PCV separator. He has a recall letter for that and I suggested he take it to Ford. The def heater code wasn't present when I checked the other code.
