A physical difference and enabler for the 275 HP rating bump in 2002-2003 manual transmission trucks was the addition of an ECT sensor that the 99-01 engines were not equipped with.
Ford stated that with ECT info being able to be monitored by the PCM, they felt more comfortable pushing the envelope on tuning (with manual transmission models) to remain competitive (bragging rights to advertise the highest available HP in a diesel at that time).
