I can't figure out what's going on. My truck rides smooth all except right between 35 and 40 mph. When I'm in that zone it shakes like there is a major wheel imbalance issue. It happens every time no matter what, regardless of if it's under a load or not. I've had the tires balanced and the alignment done. I had them re-check the balance just because it was still there and they assured me the tires were round, no bubbles, and balanced. I just don't know where to go from here? Is there a possibility that this could be a driveshaft imbalance issue where a weight fell off maybe? The driveshaft carrier bearing looks good (rubber intact and very little play) and the u joints appear to be good with no play... could probably still be bad.
Great question on the neutral question. I totally forgot to do that earlier to isolate the driveshaft. When I just took it out real quick I got it to 37 mph, it started doing its shaking thing, I dropped it into neutral and the shake continued. I'm guessing that means it's not a driveshaft thing then being the driveshaft is no longer engaged once in neutral? Is there anything else I can do to check, once again, the tires? I mean I can take them back for a third time and really emphasize my concern or is there something I myself can do to dial it in that you know of? Thanks!

Just because you kicked it in to neutral all you have done is not put it under a load, power on or off, it is just along for the ride and still spinning.
If you have checked the wheels 2 or 3 times and the suspension front & rear a few times also and are sure it is not them, how old are the shocks?
I would pull the drive shaft and check each Ujoint and the carrier bearing. A little play any where in the drive shaft is not good.
I almost forgot check the trans mount. Is it oil soaked or cracked? Motor mounts to when checking.

With the truck off, lay down in front of it and have someone go in and rock the steering wheel back and forth and observe:
- tie-rod frame and axle mounts
- tie rod bushings
- driver's side frame rail
- steering box mount
- drag-link
My guess is your tie-rod bushings and axle-mount are worn-out, possibly cracked frame under the steering box where it mounts.
Thanks so much everyone! A wealth of knowledge! This weekend I'll see what I can figure out by applying everything you all mentioned. Hopefully it becomes obvious as I work through the list of items to inspect. I'll keep you posted on what I find.
