Drivers front
Drivers rear
Drivers middle of intake
Passenger front
Passenger rear (tree)
big hose with clamp goes to brake booster
little hose goes back to transmission
Hope this makes a bit of sense.

The later trucks had a manifold on the cowl (See below 3&4) that is always an option also and can make for a cleaner install.
