Your pickup fell off in your tank (Christmas tree) so at 1/4 tank you think your out of fuel but still have four or five gallons left. Does your gauge work?
At the risk of stating the obvious, in addition to the pick-up 'Christmas tree' falling off, some other possibilities come to mind:
- Sending unit is off. One of the PO's might have changed it and installed what was on the shelf versus the correct part.
- Tank is smaller than spec. Might have come as custom order, or a PO might have done it.
- Not getting a complete fill due to the OE setup which causes a *lot* of foam.
- Combination of the above.
One way to test for total capacity is to make absolutely sure the fill was complete by dribbling fuel until there is no more foam at the filler neck and it is just fuel. Then drive until fuel level gets to the point where you would normally fill, and instead drain anything leftover. If what comes out is nominal, then the tank is smaller than usual. If another ~5 gallons comes out, focus is on the pick-up and / or sending unit.
Alternatively, just drop the tank and do the in-tank mods and fix whatever else needs addressing. If the tank does turn out that it is smaller than normal by way of a part number review, you are half way through installing a correct / larger sized one

Might be more work than you're willing to do at this point, but a 38 gal rear tank is a great upgrade for these trucks. Alternatively you can delete the shower head and add an external filter and minimize future problems in the tank. Though you'll have to use compression fittings or something similar to extend the pick up tube where it needs to go.
