Lumpy cams are caused by overlap. (General simplification)
Overlap kills power in a diesel motor.
You will be hard pressed to fnd a performance cam for the 6.9. I did a bit of a search but find not much that is current. If you find one, it most likely will not be lumpy. Longer duration, a bit more lift.
A turbo has long been the path to more power for these engines. Extra mods required for that as well. Read on:

yeah, I know it was not an IDI type engine
Some of our old Marine engines stumble and miss like a devil at Idle ... normal in the tugs we have here most all at least 30 or 40 years old
