Losing power when accelerating

I have a 2009 super duty with the 6.4 recently when accelerating it will lose power but it’s speratic and doesn’t last like it’s struggling to get fuel any ideas anyone had this problem

Fuel filters/fore/aft age of change. Any mods? Can you paint us a picture of maintenance and do you have a scan available?
