getting tired of this. It happened once before, same wheel. I replaced nuts and no more issues. Today, did the exact same thing. What gives, why does it keep doing this??
What kind of axle do you have? My gut feeling is that the area where the bearing retainer is pressed on, has become nominally too small. It could be from being polished or cleaned up after a past failure. Probably need to replace the axle shaft.
D60. This time ate the outside bearing. We swapped it out on the side of the road, now gotta order nuts and washer,, again, so I can get it back on the road.
What is your exact procedure? The nuts shouldnt be able to move once you set them.
That is what I was thinking.
There should be a lock washer of some type. Thinking there is a flat washer, a nut to adjust the bearings preload, a lock washer then the last nut to lock the first nut & washer in place.
The 2 washers I think would have a tang in the center that rides in a groove in the housing to keep them from spinning.
Some times the first nut has a pin that fits in a hole of the 2nd washer that has a tang. This set up keeps the first nut from spinning and the 2nd nut holds that washer in place.

Ha I Just liked this picture on your instagram and thought "hey I think I saw this truck on fte". Anyway read the comment I left. Same thing happened to my highboy and I replaced the adjustment nut/tabed washer/lock nut set up with a single self locking nut off a 90s 10.25. I heard they only used the single locking but on the left side though
Ypu should have a lock washer like this between the two nuts
last time, the tab on the washer that sits in the keyway was sheared off, this time the same thing. I replaced the washer last time and tightened them correctly.
One of the few who can say they literally drove the wheels off their vehicle. I saw another guy lose the whole wheel off his dodge on the freeway one night. Big rooster tail of sparks and that tire launched higher than I could see came back to earth next to me a quarter mile later. Kept rolling down the four lane for almost a mile. Bad juju.
