400 Flywheel/pressure plate

400 Flywheel/pressure plate
Having trouble finding the correct pressure plate. Can't seem to get the correct bolt pattern. I've tried 11" and 12" clutch kits. The closest has been 11" diaphragm but the holes are a little off. Looking for suggestions.

This is the ID engineering number marked on the part, it is not the Ford part number.
Ford part numbers listed in post #2.
btw: Look at D7TE-6380 again / 351M has a -DE suffix; 400 has a -CE suffix
