My King Ranch has the 20" wheels and a 18" Firestone spare. The circumference of the tire is the same on both. Ford doesn't put high dollar wheels under the trucks for spares.
At least Ford isn't putting a can of fix-a-flat (no spare) under there like Hyundai does.

For smaller vehicles, the "no-spare" option actually makes a lot of sense. Most drivers aren't gong to change a tire anyway, they'd just call roadside assistance, so a smaller a lighter emergency solution is pretty smart.
I work at a car rental company and about the only time a spare tire gets used is when it's stolen right out of the vehicle!
I'm happy to have a "real" spare on a pickup however, as with me, I'm more likely to be out of cellular coverage or towing/hauling where a full size spare is a safety issue.

My Mustang GT with the 5.0 Coyote has a spare tire, jack, and tire wrench in the trunk. And it wasn't an optional add-on.
As far as steel 18" wheels under the trucks for spares, they do that because most people never remove them for cleaning or to use them and so they get blasted by weather, road grime, salt spray, etc. constantly. Steel holds up better, is less expensive for them, and can be sanded and painted if need be by the owner later down the road.
