4.0 head freeze plug

So I was driving home the other night and the engine over heated all the sudden and steam was coming out of the hood. Lucky I was close to home. I let it cool down and put some fluid in it and it pored out the front of the motor at the head. I thought head gasket or intake. Well I took off the alternator bracket and tried the water again and it was coming from a hole in the freeze plug. I also noticed a funny round shaped object fell to the ground. It was a hard object that looked like it was the size to fit into the freeze plug to plug the freeze plug. After looking at the freeze plug I noticed that the hole was odd because the metal was good and the freeze plug was hard to get out so a tight fit. I installed a new freeze plug and I'll find out tomorrow if we're all good. My thought on this and question is why and what was the patch over the freeze plug. It looks like someone poked a hole in the freeze plug and patched it maybe looking for a future repair for some easy money. The alternator was replaced in the past so wondering if this was done then (way before I purchased it). Edit 4.0 OHV
