00 Lightning 3 rocker arms falling off

3 rocker arms fall off on the driver's side head. New lifters and still does it. Valves were done by machine shop and it starts, but the fallen rocker arm ticking starts. No engine codes.

That's weird. Are all the parts you are using new?
