Smoke!!!

Ok, I replaced the turbo last Wednesday. All went well, truck stopped smoking once the old oil burned out of the exhaust. Used the truck Thursday, Friday and yesterday. Yesterday when my guy came back to the shop it seemed to be smoking a little. Today when he got to the job, about 15 miles away, it was smoking like crazy and there was noticeable loss of power. Looks like the new turbo failed. What would cause a new, not rebuilt, turbo to fail so quickly?
