E350 rear brake help

E350 rear brake help
My work beast is a 1987 e350 duel rear wheels 7.5l with 3.5 inch rear brakes. I have done the rear brakes on this thing about 4 times now in the last year and a half. It keeps breaking the brake nails on the passenger side and of course it then grinds everything in their into nothingness. Any ideas what could be causing it?
