The fixed idler is more for helping with belt vibration. You can get away without using it but might need a slightly shorter belt. They are very difficult to find in average or better condition. They are subject to wear and I believe they are obsolete.
The 460 uses a similar arrangement and I was unable to reuse the fixed idler even after replacing the bearing in it because the shaft was ovaled. It works fine without and all the 351m 400 motors with AC I have seen do not have the fixed idler.
Yes you do need engine family specific brackets and dealer hang on is usually different. The adjustable idler is typically available at auto parts stores or replace the bearing in your used one, it is pretty easy.
