Offroad Design has been making doubler cases for the GM NP203/205 combo for years. They may make them for Ford cases also(?). The only downsides I see to them are a more complicated shifter arrangement and more things to break/go wrong (mostly in the 203).
I don't know how much use they would be behind a diesel engine. Seems 2ish:1 or 3ish:1 would be plenty of low range for most uses to me.

with my granny gear, the 1345 is plenty, but the idea is too temping hahaha
There is a secondary transfer case installed behind the primary, essentially. That way you can have either of the cases low range, or both for a combined super low range. It also allows for 2wd low.
In the case of Offroad Design's setup, the front case (NP203) is actually just the reduction box machined down to bolt onto the front of the NP205. That way it's much less bulky.
when i had the 1356 t case and ZF5 speed in my 88 it would idle slower than you can walk regular pace, and go anywhere.
i won a $100 after a guy bet me it would not climb a hill. around 65degrees and 250 feet high.
from bottom to over the top at idle in first gear and transfer case in low.
