I think I have one of the above issues, just started one day out of the blue .... changed exhaust gasket one side, but still there. It does sound like when one cylinder is firing, the noise is evident .... Not sure how difficult it would be to take a video w sound and post it ....
Could be a broken check valve in the air injection system around the back of the motor.. get a small mirror and crawl in on top of the motor and have a look down behind.
I also have this problem, but you cant hear the ticking noise unless the engine is under load(in gear). When the truck idles its hardly noticeable. When i did a seafoam treatment to my brake booster vacuum line i noticed smoke coming out of the exhaust manifold flange area drivers side, maybe the same situation for you?
