Well since this thread never got finished I thought I'd add to it so if someone else comes looking they don't end up empty handed.
The rear metal bracket or better yet called a "washer" that holds the ficm down is part # W301381,
rubber grommets part # 3C3Z-9C150-AB,
isolator bushing for grommet part # 3C3Z-9C151-AA,
the FICM mounting nut part # W302683,
the front CAC to front of ficm mount bracket part # 3C3Z-6K864-AA. I'm still attempting to find the bolts part #'s.
It appears that 2004 models they moved the rubber grommets from the FICM to the FICM mount as seen here. Is this correct? Also what is that bolt stud for on the rear bracket?
Pretty sure they're just 5mm flange head screws. Unless the intake tube is knocking against the bracket and making an annoying sound, it really isn't anything to worry about. The newer years don't have that bracket or the threaded inserts in the tube.
