I would agree with testing the air pump. If I'm not mistaken does not the air pump go directly to the front of the cat. He is complaining that the plugs are white,Lean condition. if I am wrong could someone correct me it's be some time since I worked on this vintage. To me if the on 2 sensor is up stream and the air is directed to the cat. Would this not be the problem, how could this cause plugs turning white?

Good question. I am curious myself how it all works. Maybe an expert on this stuff can chime in here. Maybe Conaski? He seems to know allot about these trucks.

I didnt end up having time this morning to do those tests before work. I will probably do it tonight after work. Pull it in the garage and get to it.

As everyone likely aware they had more than one configuration and not sure I've seen every possible combination of em! a few of them yea but doubt all of them.....maybe but doubt it.

5.8L F250 so should be air to manifolds or air to back of heads or air to both and before the O2 sensor, not sure exactly when they changed from heads to manifolds to manifolds only in the F250, or rather if its always been an either or thing.

7.5L yea one line direct to cat post O2 but includes one to exhaust manifolds pre O2 with it, one constant and one controlled via a diverter valve.

Doubt it has one direct to cat but only way to know for sure is to look at the system, follow the tubes all of them noting where each inject the air into the system and if they are pre or post O2.

The air injection pump was only used to control rich fuel mixture, mostly on start up and deceleration. It ran to rear of head at exhaust port or directly to manifold or pre cat. The air pump system was used to control rich mixture going to cat. In turn stoping damage to the cat. I do remember the k car several of them with cheer red cats because of rich mixture.
I don't think white lean spark plugs and air pump would have any connections.
If the air pump was the cause of the problem I feel the plugs would be more of the rich condition. If the 02 sensor seen lean condition it would adjust the block learn to rich value.
This would make for a rich condition not lean condition. I still feel that if the EGR system was plugged and ran KOER test, the 02 sensor would still want to see a richer condition EGR commanded. I know you did a fuel PSI test but did you do volume test. You can have pressure but a plugged fuel filter will not deliver proper amount of fuel. Again clear codes and take the truck for a road test and see what codes come up. Not just a KOER test,
