Cool 2.9 flywheel/starter grind

Ok so first off, I drive a 1987 Ford Ranger 4x4 2.9L 5speed, around 5 months ago it made a grind noise, so I installed a new flywheel, clutch, pressure plate, throwout bearing (pilot bearing), and starter. Well around 5 days ago the grinding started again and now its to the point where it will not start. What could be causing this? Starter bench tests perfectly and looks good, used flashlight to look at flywheel while having somebody manually turn the crank, it all looks good.

The clutch in the starter bendix may be bad.

The starters on 2.9s "back off" note how the upper mount hole is ovaled, loosen it, use a pry bar and drive it inboard toward the flywheel as far as it will go, re-tighten.
Seems peculiar to the 2.9, my 2.8 has never had this problem.
