Starter Whine/Grind

I have a 2001 F 150 w/ 4.6. Occasionally (3 times in last 2 months) it will grind and whine when I turn the key to start it. Apparently the starter motor doesn't engage the flywheel.

I thought maybe I hadn't turned the key firmly so will pay more attention in the future, but does anybody think I may be developing a problem or do they "all do that"?

Either a bad spot on the flywheel or the start is failing to kick the bendix out far enough.

If it's a bad spot on the flywheel, do we think it can "wear in" or is it only going to get worse? How would that happen, or is it likely a flaw in manufacturing?

It would only get worse and eventually eat the teeth off of the starter. Another possibility, the starter could be loose, making it not mesh properly with the flywheel.
