I'm a big fan of BDS hardware. If you're buying a kit from them I'd try to either opt out of shocks or sell them for Rancho adjustable sticks just because I'd like those better, but the BDS front shocks on mine are going on 40k with no issues. 4" on a Super Duty I'd do the regular kit with radius arm brackets, unless you're planning on going small with the tires so you can stuff them into the wheel wells while articulating.
My local 4x4 shop used to sell other brands and still do but they love BDS. A little more expensive but the quailty is worth the expense. Oh I would go 4 link for the better ride.

I've always heard you shouldn't go smaller than your stock wheel size, but I would say as long as you have clearance for your brakes it shouldn't be an issue.

You can downsize your wheels no problem. In fact, I recommend it. 20" wheels are so common these days, but they hurt ride quality, ESPECIALLY off road. 18" is probably good. Just remember if the total diameter of the wheel and tire changes your speedometer will be off. You can have it recalibrated though.
You can't go wrong with BDS. I used to work at an off road shop, before getting hired at FoMoCo, and we were one of the largest BDS dealers in the country. I've installed several of these lifts on full size Fords. With that said, I HIGHLY recommend the 4 link system. Its significantly more labor intensive since the rivets in the frame have to be removed, but they its a great driving system. Definitely worth the money.
I should also note that you will want to upgrade to the Fox Shocks. If you don't do it now, you will later.
I ran BDS on my Excursion, I loved it. For a big heavy truck on all leafs, it still rode great. Like others said, upgrade the shocks and you'll be happy.
Go with a 4 link 4" on 37's on 18's. That's what I'd do if I ever lift mine.
I would go with 20" wheels if you are on the road most of the time as it will help handling and sidewall flex will be minimized. Go with 18" wheels if you wheel a lot and need sidewall flex for traction and aren't too worried about high speed handling on the highway.
