I had what felt like this problem myself. I decided that the only way I was going to get more power was to add a turbo... added a used Banks kit($500) and ended up with a truck with /way/ more power.
I'm pretty sure my problem was compounded by the fact that I'm at 2000ft elevarion, which, if my math is right, has about 80% of the air pressure of sea level... so 80% of the power(because we're air-limited here).
The turbo negates that issue, so even if the engine is worn and not super great anymore, it boosts the power of my worn engine well beyond what it would have been new, stock, at sea level.
well i didnt find the problem but i fixed the issue, bought a 24v cummins for it yesterday, cummins fixed the problem i had with my other truck with a bad 7.3 a few years ago also
