To close the loop on this info, I have learned....
If you read Justin's PDF in Post #13 above, at one point it tells you to turn off overdrive with the O/D cancel switch on the end of the shifter lever. This is a VERY-important step, as the OBD Ready state will not change without doing this.
My friend wasn't aware of this switch, and hadn't done that part of the Drive Cycle, and he still got a Not Ready when he took it into his smog shop this morning. Another friend of his alerted him to the O/D Cancel function, he followed it, and waaa-laaa, he passed smog this afternoon!

This begs the question: When he followed the instruction to the S and not to the T, when he didn't pass, didn't he automatically realize the step he omitted was important?
Even though he has owned his 2000, F-350 since new, he never realized there even WAS an overdrive cancel switch. That's what his other friend pointed out to him.
