Sounds like if that bolt cracked its housing it was overtighten at some point or Lots of heat/vibration in that area too

Is it the Plastic that cracked????

I noticed my crack on the top side of threaded insert area when I removed it to send to Ed. It has never been removed since new to my knowledge. The STC was repaired by a ford mechanic back in 2010 under warranty but don't know if he would have removed it or not.
