My guess would be something with the injectors but you need to have the DTC codes read if there are any. Without any gauges and not knowing if there are any trouble codes we will just be speculating.

No one has suggested to have the FICM checked

Get the scan gauge 2 at auto zone or the manny fine retailers off the Internet .
Go to the search bar type in scan gauge and there will be hrs of reading on it.
I've got one to moniter the goings on of my engine. Unfortunately it doesn't monitor fuel PSI or EGT

A lot of guys on here like the scan gauge II, I like the Edge insight CTS but it doesn't monitor a few things the scan gauge does like IPR% fan RPM and some others, but will monitor EGT and you can monitor fuel pressure with the Edge EAS or you can go with the smartphone apps like dashboss for IOS or torque for android.

You have to add in the pyro in the exhaust for egt and the sender for fuel pressure to get a signal to these gauges. The rest comes from the obd2 port below steering wheel.

Drain water, into a glass jar, let it seperate, look at how much water, replace fuel filters, do you have any gelling of the fuel? Lots of water issues with these trucks only a little can ruin injectors fast. If this doesn't bring up anything then you need to start looking at FICM voltages and start up. I am thinking 50/50 it's ficm or injectors, like 4 of them if it's sounding that bad.

The Pump was installed in September 2012 and the EGT has been replaced in I believe September 2011 along with several other components on that trip.
