Unhappy Radiator Fan

A freind of mine replaced the water pump in my 1994 F250 7.3 ltr
a couple of weeks ago.
While i was idling the other day i noticed the engine temp gauge was in the danger zone. I shut the engine off and looked under the hood and saw my radiator fan laying between the shroud and the radiator.
What did my friend do wrong? It looks like i should be able to thread the fan back onto the stud. Should there be a lock washer there? Is there anything i should do special?

Radiator Fan

No lock washer, they have left handed threads so as the engine runs it keeps the nut tight. He probably did not get the fan clutch tight, or cross threaded it (easy to do). I would reinstall and torque it down a little, you'll be fine.
